Finally have all my information (documentation of medical supervised weight loss, psychological, recommendation of PCP and nutritional counseling) to the doctor and the request for the gastric sleeve surgery was sent off to the insurance company today.  I'm a mixture of happy and nervous.  Happy to be one step closer to surgery and nervous because I'm one step closer.
