Aetna ASA PPO?

Has anyone had this particular type of insurance & been approved for surgery? My agent says they cover any medically neccessary surgery but you have to jump through hoops. Anyone know what the hoops might be? I hope to get the sleeve.

i was approved with aetna. 3 month weight management 2 year weight history and i have diabetes

I have Aetna open access and was approved. 4 months supervised diet pre-op. BMI over 40 1 co-morbidity.
