How do I add my surgery date to top right of my gray bar info

I am trying to add my RNY date to the top right hand corner of my gray bar, that shows my profile pic, but I can't seem to figure it out. Any help is appreciated.

Lapband surgery in 2009 - Revision to RNY August 13, 2013 with gallbladder removal.

HW - (260) SW - (197) GW - (135), updated on 1-2-14 to 125lbs HT 5'5" Goal reached 3/2/14-revised goal to 120 on 3/9/14 reached 4/6/14

Hey TD ~ okay, try this ~~~

1. Go to "Health Tracker" up in the blue bar

2. On the left side, click on "My History"

3.Under the "surgeries" tab, click "edit" (the pic of the tool to the right)

4.After you put your info in, you have to make sure to check the box where it says "Primary" (right under the surgeon's name)

That should work !!!

It worked!! I had done everything except check the "primary" box. Thanks so much.
