Plastics in Oregon

Anyone have Plastics in Oregon?  If so, who with and what procedures, cost?

I was interested in a BL with BA to start with.  I have heard good things about Dr. Jewell in Eugene but just read that he will only do the BA and BL separately and 6 months apart.  I REALLY don't want to do it that way if all possible.
