I signed mine 5.5 months before surgery and was never asked to sign another. I signed prior to being given my surgery date and when they called with one, I didn't have to sign anything else. I didn't go through Ottawa but am sure that part would be the same all over. You could always call them to inquire and then it may make them realize you still don't have a date.

I finally got the call - June 5. :) Thanks everyone.
I was advised, however, that since this is elective surgery that if a trauma surgery happens on June 5 then I will be bumped and rescheduled within 3 weeks. There was someone who couldn't have their surgery yesterday because there was an emergency trauma surgery that had to take place.
