Cost of surgery vs cost of obesity

I cannot find that post. I am putting together a packet to send with my doctors referral and would like to see this paper. I'd like to make one showing surgery vs cost of my medical expenses without surgery. Any ideas on what I should put in my packet? I'm searching the internet and WLS site for ideas. I'm hoping my packet will be so powerful they won't be able to say no. I have no documented co-morbidities. I am 5' 6" 256lbs and have stress incontinence, pain in my knees, ankles and back. I have a small leak in my heart possibly from takin phen-fen which my doctor at Group Health prescribed. I cannot do low impact aerobics without injuring my knees and ankles.
