Question:
Please help, I can't find a letter I read here once, and I need it!!!!

A little while back a found a sample letter that someone had posted that was from their psyc evaluation. I am having surgery on Dec. 9th. and need one. I have someone to do it for me, but he does not do them on a regular basis and ask me to show him the sample that I mentioned. Misty, I think this will be helpful. This is a link to my doctor's letter to Psyc evaluator. It tells him what needs to be covered in the letter. Once he has that, it should be realy easy to write. http://www.thinnerfuture.com/steps_surgery/letter.htm

I needed a psych eval to get my surgery approved and all I did was call the insurance customer service to get it authrized through their behavioral health department, and they refered me to a local psychiatrist. He gave me the standard MMPI test,(560 of the most retarded questions you have ever answered) and interviewed me about 30 mins, and it was done. My surgeon or I never needed a letter. Hope this helps!
