Question:
PCP letter: medical clearance and comords?

Sorry for all the questions but I am getting ready for a round of appts and need to construct the PCP letter. I need a letter from my PCP to my surgeon that has my history, comords and clearance. What should the language of the clearance be? that I am in good health? How can that be with so many comords? Just curious how to balance this. When you come to this web site. There are the little foot prints that will track your WLS journey. Click on the foot print that states waiting for insurance. There is a list of do's and don'ts and they have sample letters. It is very helpful. I used it to start the process with my family doctor. She was very impressed!
