Does anyone have a letter from chiropractor and psychologist I could use?

I am not sure what you are asking for, but you need to see your own psychologist and chiropractor for a letter. These items cannot be used interchangeably between patients with the name changed to your own...

Obviously I worded this question wrong. I already have an appointment with a chiropractor and psychologist and dietition and podiatrist. I wasn't planning on using the letter as my own. I just was curious as to what it looked like, said and kind of what to expect. Let's just forget it. Thanks
