Question:
What exactly do I write in the letter to my insurance for authorization?
I need to write a letter to my insurance for authorization, but I am not sure what to write. Please help; I don't what to leave anything out. — Claudia (posted on January 2, 2006)

I didn't have to write a letter my physician did. Are you sure you are
supposed to?

I did not have to write a letter into my insurance company, but I think
they would want to know about your health problems. How long you have had
these problems. What things you are being hindered from doing because of
your heatlh. Have you had at least six months of a Dr. supervised weighlt
loss plain. If so, I am sure they may want that information as well. Most
surgeon's offices have someone in the office do the letter. Make sure
before you start that you are in fact the one who is supposed to do the
letter.

I'm pretty sure that you need to give any info to your pcp or surgeon that
you have contacted that he does not have about diets you have been on and
he is supposed to write the insurance. The surgeon is usually the one to
ask for authorization.

Hi Claudia - My doctor's office handled this letter. Are you sure you have
to write it? The doctor should know exactly what to put in the letter.

I work for an insurance company, and if the surgery is covered by your
insurance plan, the surgeon's office should be sending the letter to the
insurance company. Afterall, if the surgeon wants to get paid, and they
need authoriization, they should have someone in the office that makes sure
this gets done and not leave it to the patient.

I had a denial my first time around (Cigna) and I sent in a letter going
over all the symptoms I have due to being overweight, plus the fact that my
regular MD and two specialists have stated I need the surgery. At the end
of the letter, I told them that if I did not get a reversal of their
decision with 5 days, I would seek the advice of an attorney experienced in
these matters. Boy, did that get the ball rolling! ; )
They set up a phone conference with my surgeon and invited me to attend,
but my surgeon said he would prefer to handle it himself. They approved it
that same day. I am having a resection. I had staple bypass 20 years ago
and am having severe reflux and regurgitation because of it. I having a
RNY.
