Consultation-What criteria does Ucla look for?

I am waiting for a consultation with UCLA's Bariatric Program. My BMI is 46 & a few co-morbidities -- herniated disks, foot & ankle pain, & shortness of breath & a few others that I don't have documentation for. I am constantly told I don't look my weight (245 at 5 ft 1 in.) Yet, God as my witness, I feel my weight. Clothing can only hide so much. Yet I'm afraid the surgeon may not "ok" the procedure because I don't "look" big enough. I think I look like an apple with toothpicks for legs & arms & attempt to guise the rounded apple portion (aka belly)with carefully picked out shirts. Does anyone know what criteria the surgeons at UCLA are looking for?
