Behaviorist & Dietition: April 17th, 2012

2nd Information Session: Spetember 19th, 2012

Endoscopy: October 5th, 2012

Surgery Consult Dr. Raiche: October 16th, 2012

Surgery With Dr. Yelle:  December 24th, 2012

I'm sure I'm missing an ultrasound in there between the 2nd info session and Endoscopy but that's pretty much the gist of it :)
